Emergency Diversion Mobile Respite Case Manager
The Emergency Diversion Mobile Respite (EDMR) Case Manager operates under the Mobile Crisis Intervention (MCI) Service within the Community Behavioral Health Center (CBHC).
In this vital role, the EDMR Case Manager will collaborate with the MCI team, the Department of Mental Health (DMH), and local community partners to support individuals at risk of emergency department visits, inpatient psychiatric hospitalization, and continued involvement with law enforcement and Mobile Crisis Intervention.
The goal of this role is to promote diversion from emergency services by connecting individuals with appropriate community-based treatment, supports, and resources that foster stability and recovery.
The EDMR Case Manager will:
- conduct comprehensive psychosocial needs assessments to identify strengths, barriers, and service needs
- develop individualized service plans
- coordinate services
- facilitate access to community-based resources and treatment
- collaborate with providers to address barriers to care
Schedule: Full Time, 40 hours
Pay Rate: $25.50/hourly

Required Skills
- Strong organizational skills and the ability to multitask, appropriately prioritize and manage multiple competing demands
- Excellent verbal, telephonic, in-person and interpersonal communication skills required
- Intermediate computer fluency, including Microsoft Office
- Valid driver’s license, safe driving record and regular access to a reliable vehicle
Required Experience
- Bachelor’s degree in Psychology, Social Work, or related field
- Experience working with or understanding of Mobile Crisis Intervention Services preferred
- Two years' experience (including internship) working with adults in behavioral health and/or substance use settings, with experience in case management and knowledge of the adult system of care strongly preferred
